REGISTRATION INFORMATION

Campers’ grades are determined by their grade level during the 2007-2008 school year.

Enclose a $50 deposit per camper for most programs. Balance is due upon arrival. If you cancel at least 14 days before the scheduled program, all but $25 will be refunded.

Enclose a $15 deposit per person for all weekend programs ($30 total for a child and adult).

Campers are placed in cabins according to sex first, then roommate preference, and finally by age.

Because our cabins can house 10 campers, groups of friends consisting of 6-9 campers are difficult to accommodate and may by split into two smaller groups for housing purposes.

If you are willing to accept them, please list a second and third choice of camp program.

Upon receipt of your registration, Camp Lakeview will send a health form and a letter of confirmation.

Print out the form above and mail with deposit to Camp Lakeview.

For further information or to receive additional forms, call the camp office at 812-342-4815.
